pediatric epididymitis Question

My 7 year old has tentatively been diagnosed with epididymitis. We live in the arctic and the imaging on the ultrasound can not rule out torsion but his physical symptoms do not demonstrate torsion. His rt testicle if red and swollen and it is very sore to touch but he is still playing and goofing around so the pain is not acute unless there is contact with the scrotum.He has no fever. He does not have UTI although there was a little blood in his urine. He has an explained tenderness in his lower back rt side. The day before he complained of pain in his testicle, he had a very sore throat.(not mumps, either)This is the fourth day of pain and swelling. It has not gotten worse or better. My question is does it sound like epididymis to you? What other causes should be investigated?Could it be torsion of an appendage. What questions and tests should I ask for from the doctors here? Could this indicate an abnormality of some sort> He had an inguinal hernia as a child and the ultrasound did show a hydrocele. Could these contribute? Do you know of any alternative therapies to help relieve the symptoms? Thanks so much.
