Have you tried Levaquin? It's a later generation quinolone, and kills a wider variety of bugs, and it's a little less likely to have side effects, at least for awhile.

If you are infected, 2 weeks is not nearly long enough. A urine culture (not urine test) or semen culture might help determine what you have (though they're more likely to be positive if you haven't been on antibiotics for at least 2 weeks).

If you are NOT infected, you're wasting time not checking other things -- varicoceles, muscle spasms, tiny hernias, etc.

I don't know your history, but lots of things cause testicular pain, including some spine problems.

Maybe time to get a 2nd opinion, or 3rd or 10th -- whatever it takes, especially if a lot of the possibilities haven't been checked out.
