I am a retired doc and will offer info for general, information purposes only...urologically speaking, for those of you with painful ejaculation, it can come from many sources: prostatic inflammation...as in an infected gland. Antibiotics should ultimately provide relief, but therapy may take some time. Post-inflamatory disease of the epididymis...as in post orchitic pubertal/postpubertal mumps infection...can cause what appears as epididymal casts...4 to 6 mm long cylindrical gelatinous light amber fragments that can be uncomfortable as they traverse the vas deferens during ejaculation. These agglutinated clear, yellow, non-bloody casts are like a "mold" of the inside of a section of the epididymis. As the smooth muscle of the epididymis and vas contract during ejaculation, these "hunks" of "gelatin" are forced along the narrow vas deferens, through the urethra..this can be mildly painful. I had a few patients with this condition...very few articles on it...and frequent ejaculation is helpful as it dumps the load of ejaculate before the gelatinous casts have a chance to organize,dehydrate,and become more dense and hard, making ejaculation less painful. This information is empirical and the fact that more frequent ejaculation helps is not well documented. All of you guys who had mumps at puberty and developed a swollen testicle (or those who developed sympathetic orchitis)...both sides...keep an eye out for testicular lumps, as a badly damaged/scarred testical can occassionally become cancerous. You have probably had these casts all of your adult life and have gotten used to the "pain" as simply an intense part of the ejaculatory experience. If the ejaculate becomes bloody, or a testicle develops a lump, see a urologist right away. Other causes of ejaculatory pain are varicocele ( a varicose vein surrounding the epididymis), epididymitis, post surgical complications, and a few more. Enough for now...until later. RAChatterjee |
